Sample Reports
Grantees, especially smaller organizations, are sometimes uncertain about what
constitutes a proper report. The best course is to contact your funder if you have
any questions about their particular requirements. We hope that the two sample
reports will help. They are adapted from actual reports received by the Fund --
names have been changed, and some deletions and other edits have been made. At
the end of each report are some comments about topics that would have
strengthened the report.

1. Report from The Townville Library, prepared by A. Librarian
The library is very grateful to the Fund for a grant of $5,000 for children's
programming. The board of the library decided to spend approximately $1,000 on
books, $500 on video tapes, and $500 on computer CO-ROMs.
The first task was to make a list of Vermont authors for young readers. We felt our
library did not offer an adequate selection in this area. These books were ordered
through a local bookseller.
Other books were ordered based on librarian and patron requests. These requests
included Marc Brown's Arthur books, Roger Lea MacBride's Rocky Ridge Years
series, Laura Ingalls Wilder's The Laura Years, a six-book set of My Easy
Readers, and were ordered through a book distributor.
Prior to this grant, the library did not have a children's video collection. We made
two sets of video purchases, one in August and another in November. The library
now has a strong core collection, including a very popular 18 tape set of the Magic
Schoolbus series, 8 National Geographic "Really Wild Animals" tapes, 12 Arthur
videos, and 3 of Maurice Sendak's Little Bear series.
There had been some discussion in the library of trying to connect to the Internet,
but because we are only open eight hours per week, we decided a better use of
the grant funds would be to purchase CD-ROMs for our existing computer.
Copies of invoices for all purchases are attached.
Having the Chasdrew grant made it possible to focus on resources we were
lacking in, what areas we could improve, and what goals to set for the future of the
library. We completed several purchases we had identified as goals.
Once again, thank you for your support. Our library is a more exciting place, and
more important, there are many very eager, happy children walking through our
door.
Comments: This is a good basic report. Questions the Trustees wanted
answered were: How many children use the library? How many times are the
new materials circulating? What CD-ROMs were purchased, how often are
they used, and by whom?
2. Report from Northeast Kingdom Children's Organization, Prepared by
O. Coordinator
Two peer outreach workers were hired: one for 25 hours per week, and one for
10 hours per week. Their initial training included : professional and personal
boundaries; confidentiality; ethical decision making; reporting; outreach
curriculum; and how to identify and reach our target population.
The person in the 10-hour position left after three months. The other person is
taking on those responsibilities temporarily. We have placed advertisements and
posted flyers to try to hire a replacement. We have a few interviews scheduled
and hope to hire a new person in the coming month.
Our activities this quarter included:
A Youth Honoring ceremony. This event celebrates the accomplishments of the
youth over the past year.
Working with "Kids Against Tobacco" to plan a dance for youth. The dance
attracted over 112 youth. There were several speakers (a local pediatrician and a
cancer survivor) who spoke about the negative effects of smoking.
Establishing a relationship with the local health education center to develop and
create increased access to health information for youth. We would like to
establish a health resource library.
Upcoming activities include:
Holding a series of activity days at local low-income housing areas.
Holding free barbecues/concerts at a nearby lake, providing transportation for
youth.
Planning for fall and winter activities, including another dance and several holiday
activities, such as a haunted house, Thanksgiving dinner and holiday workshop.
Continuing to reach youth through direct outreach.
Comments: This report needs to answer some additional questions, such
as: why did one of the workers resign? What are the specific
responsibilities of each worker? How are events publicized, to reach the
target community? And, very importantly, what is the involvement of the
target community in developing, planning and carrying out activities?
